      and that is left me puzzled enough to ask the questions that I have posed.

      Was it a tragedy? Unquestionably.  Were there failures, in the security arrangement, in the communication, in the response? Possibly, but nothing that I have seen so far have definitively said so.  We need to wait for the facts to be disclosed, and then determine if there were mistakes and if so what we do about them.

      Incidentally, I am not unfamiliar with these kinds of situations.  Three times in my career I have been ordered to evacuate a country - twice by the USG and once by the UN.  I refused both times with the USG for different reasons, and complied with the UN order because they pulled their entire contingent out, which would left my presence superfluous.
