    Because though it no longer reflects reality (if it ever did), the notion that doctors make enormous amounts of money in private practice, and are motivated by that expected pot of gold, seems to live on in the popular imagination.

    It hasn't been true (in most areas, and for most specialties) for at least a decade, probably more.  The combination of skyrocketing expenses (for education, for insurance, for pure practice management things like the paperwork demanded by different insurers) and falling reimbursements has meant that while most doctors do reasonably well, very few are raking in those fabled stratospheric incomes.  (Though hospital management/administrators may be, and insurance executives certainly have been.)  People going into medicine now know that, and have for years now.  The argument is based on a set of assumptions about the world that's just plain wrong.
