    the late 1980s were among the very last hold-outs in the state against being forced to join the new Health Maintenance Organizations. And at that time they sold it to us secretaries as "nobody will ever have to deal with money at the door." But they found out immediately that lots of people were getting lots more care under that situation but doctors were not getting lots more money. So they invented a thing called "co-pay" which was to be a "token" fee to remind Moms they had to come up with $6.00 if they had him look at all three kids they showed up with for the same sore throat. But that was not enough to keep people away, so they raised it to $5, then $10, then $20, and now to the point that people are paying for their premiums and still paying what it ought to cost for an office visit in the form of a co-pay. Suckers. (Sorry, people, but the public's been had on this account.)

    Just in case anyone wants to stroll down memory lane to these early days, there's a book called Bag Balm and Duct Tape that captures the era and the mood.
