    Provisions in the ACA encourage Primary Care Providers to ask specific questions about depression, drug and alcohol (as well as nicotine) and exercise and safety. In a PCP visit and maybe more often in an OB/GYN visit, a patient will reveal abuse she is experiencing. At our FQHC, the person is referred to in-house. LCSW. Whence able, most of the time, the PCP will go the the LCSW and do a warm hand-off, increasing the likelihood that the patient will come in for an Intake. During the warm hand-off, the LCSW will explain what resources are available and where to call. I have had situations where the patient was with me and we had police come through the back door to take the woman to the shelter as the perpetrator was in the waiting lobby. True to form, when he saw the police car, he got in his car and took off.

      However many areas lack an FQHC and the resources to form a 501C3 to file the necessary grant paperwork.  Then there is the problem of only a limited number of FQHCs being approved each year.  Reasonable access to mental health services remain a problem in many rural and urban areas
