    In the 1980s, an Air Canada jet ran out of fule mid-flight because the fuel tech and the pilot were not on the same page with metric vs imperial measurements.

    To compound the problem, the plane's fuel gauge readout was malfunctioning at the time (not considered a critical issue) and the airline's responsibilities checklist had not been updated after the elimination of flight engineer on newer aircraft, so there was no clear procedure for validating that the airplane had sufficient fuel for its trip.

    (The pilot successfully flew and landed the plane as a glider and there were no fatalities or serious injuries.)
