    especially when you have to transcribe hand written material, or material that doesn't capture well by OCR,

    There are all kinds of spoken formatting commands.

    "New line"
    "New paragraph"
    "Tab key"

    But it does take time to train it, especially for material that has a lot of technical words and acronyms. And it still has trouble with some words, (to, too, two, 2), (they're, their, there). I fix those when I edit, but one could program a way to speak them, such as 2 = "number 2", two = "number two, spelled out", too = "t double oh", "to, preposition".
