It is generally believed that the world's most extreme airport is Lukla airport in Nepal, which has the minor problems of extreme high altitude, short runway, surrounded by mountains and unpredictable weather.
Lukla is an altiport, which means that instead of being flat, it is built on a slope, usually with a ski jump at the lower end. This means that you land uphill and take off downhill, irrespective of the wind direction.
I have never flown in or out of Lukla, but in a list of the 10 most extreme airports, number 7 was Courchevel altiport which I have flown in and out of several times.
