Disclaimer: Not being a behavioral neurologist my observations of Trump which suggest a degenerative cognitive disorder are that of a layman. As a psychotherapist my observations of his psychopathology are based on 40 years of clinical experience.
With Trump we have an individual who as president presents with a combination of policies and pathology so dangerous to our democracy that I am at a loss to find the words to describe how bad it is.
Between him already being a malignant narcissist, something almost all psychotherapists willing to go public with their analysis of him agree on, he is the most unprepared president in history.
He is ignorant of history, all history, not just the history of past presidents.
He is not able to think of long term consequences of his decisions, thus being like a four year old child.
Worse perhaps, he is an autocrat who is attracted to other autocrats (some murderous). Any empathy he shows is theatrical. He has no empathy, a primary symptom of the less severe narcissistic personalty disorder, even worse with malignant narcissists.
He is now and apparently has always enjoyed being a bully, which makes him a sadist. Aside from his exhortations encouraging violence at his rallies, most recently he congratulated batterer Greg Gianforte for winning the Minnesota election. As Robert Reich says “more generally and menacingly, Trump has licensed the dark side of the American psyche. His hatefulness and vindictiveness have normalized a new meanness in America.” AltNet
In fact, his liking to surprise women by grabbing their private parts make him a psychosexual sadist.
It goes without saying that he has very poor impulse control, of both words and behavior. His pushing the Montenegro prime minister at the NATO summit is just a recent example.
He doesn’t know what he doesn’t know (and he believes there isn’t anything he doesn’t know that is worth knowing).
For a president it is unfathomable how he NOT curious about anything.
No we get to worse case scenarios.
Because he is a malignant narcissist if things don’t go his way may become psychotic. These people, and in fact this isn’t a common disorder, so it hasn’t been studied that much, be if plain old garden variety people who have narcissistic personality disorder find their defenses shattered, that can lose touch with reality and become delusional.
Trump already has an inclination to believe conspiracy theories that defy common sense. He could go full-blown paranoid — in other words psychotic.
Now add to all this the indications others have noted that he may be showing signs of early dementia or Alzheimer’s.
Another way to put this is that on top of everything else that makes him unfit and dangerous to be president, he may be losing his marbles.

Paris Accords
No surprise here, Trump was Trump
He did it, and on MSNBC hosts are asking and answering the question “WHY?” Because, they say (and I agree), it was because the NATO leaders disrespected him. He must have actually be surprised they didn’t bow down to his bloated glorious self. This is his macho posturing for the world. He is strutting his defiant ass in front of them… No! Wait, literally he doesn’t strut, he rides behind them in a fact golf cart.
At home he is showing he is the dictator and doesn’t care that the large majority of the country believes in climate change and that we should be members of the world community in fighting it. He is even going against the major corporations.
Trump’s autocratic dictatorship disregards and defies corporate America. So much for the corporatocracy.
