Individuals should use a password manager and a password generator, and change their master password regularly. You should be able to remember your master password, so it can't be random gibberish.
Those who can, should use two-factor authentication: Something you know, like a password, and something you have, like an authentication token. Some banks offer this to their customers.
Backup early and often.
